Yes, there’s a $125 enrollment fee for a DPC membership. This fee includes a set of initial routine labs.
No, you do not have to sign a contract and you can cancel at any time. We require a 30- day advanced notice to cancel. Once you cancel, we will provide healthcare services to you for 30 days after cancellation and to give you time to establish with another medical provider.
Yes, the enrollment fee includes a set of initial routine labs. Any needed follow-up labs for monitoring of any health issues will be the responsibility of the patient thereafter. We will provide discounted pricing for follow-up labs or any labs outside of the initial routine labs you might want to have done.
At this time we are accepting patients by appointment. However, if we are at the office seeing other patients and you walk-in as a sick visit we’ll be glad to fit you in the schedule. Please expect a longer wait time in these cases however. We also offer Telehealth visits and would be happy to see you on this platform. We advise that you purchase our DPC kit for these types of visits to better assess you during these situations. The DPC kit is $99 for established patients and includes a thermometer, pulse ox, ear and throat camera, a rapid home covid/test kit, and acetaminophen or ibuprofen to take as needed. We are happy to ship the DPC kit to you if you’re not local for a shipping and handling fee.
We offer point of care diagnostic testing at our office like rapid covid/flu, rsv, strep throat, uti, and urine pregnancy and will provide you with same day results. We are partnered with local diagnostic centers for any x-rays, ultrasounds, CT scans, or MRIs as needed. For any emergencies we advise you to call 911 of go to your nearest ER then schedule a follow-up appointment with us. For any specialist, we can refer you locally as needed. If you require hospitalization you will be monitored and observed by the hospitalist physician on call. Then you will need to schedule a follow-up visit with us after you are discharged home.
Absolutely! We offer Telehealth visits for simple cases or uncomplicated follow-ups. We do recommend that your initial appointment be in person so that you can meet the medical provider and more information can be obtained like your vital signs. As mentioned earlier, we recommend that you purchase a DPC kit to have at home and utilize as needed for whenever you need to do a Telehealth visit with us.
We’d be happy to make house calls as long as our office schedule allows us the flexibility to do so. We only offer house calls locally at this time. There is a fee of $40 for house calls over a 10 mile radius and up to a 20 mile radius. Anything over that, we recommend a Telehealth appointment. If it’s after hours there’s an additional fee of $100 for our medical provider’s time and travel within the mentioned allowed radius.
We are a cash pay healthcare facility and therefore do not need to submit any claims or share you medical records with your or any other insurance companies. If you want to pay for any services using your hsa/fsa card and need an itemized invoice we’d be happy to provide you with a superbill. We will not share your medical records voluntarily to any government unless it is a life threatening or death situation and is deemed necessary. We will only share your medical records to other healthcare providers if you are being referred to a specialist, you are transferring to another healthcare facility, or you request us to do so. At Mosaic Health & Wellness, we abide by the HIPPA guidelines.
Yes, you can. We will provide you with refills on your chronic conditions to your local pharmacy. If it has been more than six months since you have been seen in our office or by Telehealth you will need to schedule an appointment in order to receive a refill(s). We do not offer pain management of controlled substances at this time. But we can refer you to a pain management specialist if needed.
It is recommended that you enroll as a member for unlimited and quality one on one care with our medical provider. If you are not a member or do not want to be a member we’d be happy to serve you with our non-membership pricing. Non-member new patient sick visits are $125 and non-member established patient sick visits are $65. We also offer yearly physicals for $150 for established patients that are not members and for $200 for new patients that are also not members. The yearly physical pricing for non-members includes a set of initial routine labs.
