Mosaic Health & Wellness Direct Primary Care (DPC) Membership

No co-pays. No insurance. Unlimited Visits.

We accept children ages two and older at this time. The enrollment fee for ages 18 and under will include a medical kit to help facilitate any sick visits in the event Telehealth visits are needed for after hours or when parents are not able to bring the child in to the office. The kit will include: a thermometer, pediatric pulse ox, ear and throat camera kit, home test kit for covid and flu, and acetaminophen to help for fever or pain. Established ADULT patients may purchase this kit as well for $99 plus shipping and handling (if not local) and an adult sized pulse ox and thermometer will be accommodated.

We are happy to provide child check wellness exams as indicated. We do not offer vaccinations at this time however we will happily provide references to where you can take your child(ren) for their vaccines. Sports physicals are an extra fee of $45 per child because it requires more time with the provider.
